The Medicines and Healthcare products Regulatory Agency (MHRA) is the government agency responsible for ensuring that medicines and medical devices work and are acceptably safe.
Together with the Commission on Human Medicines (CHM), they run the Yellow Card Scheme which collects reports of suspected adverse drug reactions (ADRs).
Everyone is encouraged, including patients, to use the Yellow Card Scheme to report suspected ADRs and help the MHRA identify potential safety issues. The MHRA then work to ensure that the benefits to patients outweigh the risks, and take any necessary regulatory action if there is a problem.

The MHRA would like to notify patients that unfortunately, due a recent problem with the Yellow Card post delivery service some Yellow Cards have been returned to sender by mistake.
If any patients have had a Yellow Card returned to them or not received an acknowledgement letter, please resend the report.
